| How to Get Started………….
Have insurance already?
- Fax or email us a copy of your latest health,
life and dental and disability insurance statements.
- Agent of Record notice will be created by
Chick Insurance.
- Copy the Agent of Record notice on your letterhead
authorizing Chick Insurance Agency to represent
you or your business.
- Fax a copy to the insurance company and send
us a copy while you’re at it.
We’ll be notified when we can represent
you or your company – that’s when
we go to work!
No insurance?
- Go to the Request for Proposal icon and complete
the required information.
- Your request will be handled by an experienced
agent who will be in contact with you personally.
